Case Management

Our case management offers individualized, comprehensive and long-term support to clients so that they can better meet multiple complex needs.

All clients are immediately pre-screened by a case manager prior to his/or her enrollment into the program. A Case Manager will determine whether an individual seeking services is eligible to enroll in a particular program depending on receptiveness and eligibility to the services being offered.

A Case Manager will also help program participants create a goal-oriented service plan, guide them through necessary services, and hold them accountable as they move toward a better life.
